تعداد پزشک عمومی در کشور و امکان اجرای طرح پزشک خانواده از ُبعد نیروی انسانی
General practitioner supply: family physician program and medical workforce

وزارت بهداشت، درمان و آموزش پزشكي متعهد است كه تا پايان برنامه توسعه چهارم (تا ابتداي سال 1389)، طرح پزشك خانواده را در كل كشور به اجرا درآورد. در حال حاضر، اين طرح در روستاها و شهرهاي زير 20 هزار نفر جمعيت در حال اجرا و در مرحله گسترش به شهرهاي بيش از 20 هزار نفر جمعيت است. انجام تعهدات وزارت بهداشت به تأمين منابع مختلف، از جمله نيروي انساني نياز دارد. از اين رو، گسترش طرح در بخش ديگر از جمعيت كشور بررسي تعداد پزشك عمومي را ضروري ميسازد. در اين مطالعه براي برآورد تعداد پزشك عمومي موجود، از مدل جمعيتي ماركوف و براي برآورد تعداد پزشك مورد نياز طرح پزشك خانواده از مستندات موجود و آمار جمعيتي استفاده شده است. تعداد پزشك عمومي موجود در سال 1388 معادل 37977 نفر برآورد شده است. با فعاليت 9842 نفر از پزشكان عمومي در ُپستهاي ستادي، 28176 نفر از اين پزشكان امكان ارائه خدمات باليني را دارند.<BR>اجراي طرح پزشك خانواده در كل جمعيت كشور نياز به 32769 نفر پزشك دارد و بر اين اساس ادامه اجراي اين طرح تا پوشش كامل جمعيت كشور با كمبود حدود 4593 نفر پزشك روبرو است. در مرحله اول اجراي طرح در زمان كفايت تعداد پزشك عمومي، جذب پزشك مورد نياز طرح ممكن نشده است. <BR>اختلاف عرضه و تقاضا بر اساس شرايط فعلي طرح نشان ميدهد كه ادامه اجراي طرح پزشك خانواده به بازنگري جمعيت تحت پوشش پزشك خانواده، شناسايي و رفع موانع اجرايي جذب و نگهداشت پزشكان نياز دارد و اين موارد از اولويت بيشتري نسبت به تربيت و تأمين نيرو، برخوردار است.</span>

<P>Objective(s): Iranian Ministry of Health and Medical Education (MOHME) decided to implement Family Physician Program (FPP) all across the country in 2005. Workforce supply is among essential factors in most implementation projects. The aims of this study were to estimate General Practitioner Supply in Iran, and workforce adequacy analysis of FPP.<BR>Methods: This study estimates "General Practitioner Supply" based on stocks and flow model and "Required General Practitioner" based on the policies, health care delivery programs and population.<BR>Results: Implementation of FPP for whole population of Iran requires 32769 practitioners while according to the results of this study supply of general practitioners are about 28200.<BR>Conclusion: General practitioner supply is especially important in implementation of FPP. Currently, the program is implemented at rural level and in the cities with less than 20000 populations and the following step is expanding it throughout the country.<BR>The results of this study show that there is an obvious shortage around 4600 in general practitioners. Moreover the current experiences demonstrate that the recruiting and maintaining of general practitioners have not been successful in recent years. However, FPP expansion needs reassessment of population patterns receiving the service, and requires innovations to recruite and maintain practitioners.</P>
