وضعیت اجرای طرح "نظام ارجاع و پزشک خانواده" از دیدگاه ارائه دهندگان خدمات
Contract performance and training conditions in family physician and referral
system: the providers’ perspectives

در سال 1384 وزارت بهداشت، درمان و آموزش پزشکی، وزارت رفاه و سازمان بیمه خدمات درمانی اصلاحاتی را تحت عنوان «طرح نظام ارجاع و پزشک خانواده» در سیستم ارائه خدمات بهداشتی کشور مستقر نمودند. این طرح در این مرحله برای تمام روستاهای کشور و شهرهای زیر 20 هزار نفر جمعیت اجرا شد. سیستم عقد و اجرای قراردادها (یا احکام) در رابطه با ارائه دهندگان خدمات، نقش بارزی در استمرار اجرای طرح دارد. و وضعیت ارائه آموزش برنامه پزشک خانواده در ارتقای عملکرد پزشکان و ماماهای شاغل در این طرح نقش به سزایی دارد. این مقاله به بیان وضعیت موارد ذکر شده از دیدگاه ارائه دهندگان خدمات شامل پزشکان و ماماها پرداخته است. نوع این مطالعه مقطعی (Cross – sectional) بوده و در سطح کشور انجام شده است. افراد مورد مطالعه شامل 1780 پزشک و 1772 ماما بودند. حدود 60 درصد پزشکان و همین حدود ماماها در مراکز روستایی ارائه خدمت کردند. یافته ها نشان داد که به حدود62 درصد پزشکان و 72 درصد از ماماهای مورد مطالعه، قرارداد یا حکم حقوقی تحویل داده شده بود. تنها 47 درصد پزشکان و 58 درصد از ماماهای مورد مطالعه ذکر نمودند که میزان دریافتی آنان مطابق قرارداد است و فقط 20 درصد پزشکان و 32 درصد از ماماها زمان پرداخت را مطابق قرارداد ذکر نمودهاند. حدود 15درصد پزشکان و ماماهای ارائه دهنده خدمات هیچگونه آموزشی نداشتهاند و سایرین نیز غالباً در ضمن خدمت آموزشهایی در این زمینه داشتند. حدود 80 درصد پزشکان و حدود 76 درصد ماماهای مورد مطالعه شرح وظایف خود را مشخص ذکر نمودهاند. کمتر از 60 درصد پزشکان و 41 درصد از ماماهای مورد مطالعه برنامه مکتوب را در اختیار داشتند. به نظر میرسد ارائه مداخلات و اجرای آن در زمینه بهبود شرایط عقد قرارداد با ارائه دهندگان خدمات و نیز ارتقای آموزشی آنان از ضروریات تداوم برنامه پزشک خانواده است.
Objective (s): In 2005, Iranian Ministry of Heath, Ministry of Welfare and Medical Services Insurance Organization conducted ‘Family Physician and Referral System’ (FPRS) as a major reform in heath system. The first step of the FPRS was carried out through all rural areas and small cities with population less than 20,000. The aim of this study was to assess the providers’ (physicians and midwives) perspectives about the contracts and training conditions in this program.<br>
Methods: This was a cross-sectional study using a designed questionnaire including items on contracts and training.1780 physicians and 1772 midwifes were studied through out the country.<br>
Results: About 60% of physicians and midwives were working in rural centers. About 62% of physicians and 72% of midwives had a print version of their contracts. About 47% of physicians and 58% of midwives indicated that that the amount of payment was similar to the contracts. Only about 20% of physicians and 32% of midwives reported that payment was timely as indicated in the contracts. About 15 percent of providers indicated that they had no training course about the FRPS. Only about 60% of physicians and 41% of midwives had a print version of the program<br>
Conclusion: The findings suggest that for improving ‘family physician and referral system’ and its proper continuation and expansion improvement of contract conditions and training courses are essential.
