اپیدمیولوژی سوءمصرف مواد مخدر در رانندگان وسایل نقلیه عمومی برونشهری کشور
Epidemiology of drug dependence among commercial drivers in Iran

رانندگان وسایل نقلیه عمومی بهعنوان گروهی پرخطر در سوانح ترافیکی شناختهشدهاند و ارتباط واضحی بین سوءمصرف مواد مخدر و سوانح ترافیکی نشان داده شده است. هدف این مطالعه ارائه الگوی اپیدمیولوژی وابستگی رانندگان وسایل نقلیه عمومی به مواد مخدر در ایران می باشد. در این بررسی با استفاده از یک نمونهگیری صید-بازصید، بهطور تصادفی میزان شیوع سوءمصرف مواد تعیین شد. در کل 1506 راننده بهطور تصادفی و به نسبت انواع وسایل نقلیه عمومی انتخاب شدند. برای تعیین سوءمصرف از آزمایش سریع ادرار (آکون ده تستی برای غربالگری) استفاده شد. مصاحبه گران آموزشدیده به همراهی پلیسراه اما بدون حضور مستقیم آنها آزمایش را در ایستگاههای بازرسی گرفته و پرسشنامه مربوطه را تکمیل کردند. بهطورکلی نتیجه آزمایش 14/1 درصد از رانندگان مثبت بود. سه ماده مخدر تریاک (55 درصد)، مرفین (21/3 درصد) و متادون (8/5 درصد) بیشترین شیوع را در بین رانندگان داشتند. مدل رگرسیون لجستیک اختلاف آماری معنیداری بین معتادان با سطح تحصیلات را نشان داد. رانندگان تریلی (21/5 درصد) و کامیون (15/1 درصد) در مقایسه با سایر رانندگان میزان شیوع بالاتر اعتیاد به مخدر را نشان دادند (P<0.001). بیشترین میزان مصرف مواد در شبهنگام (44/3 درصد) و در مسیرهای طولانی (26/8 درصد) گزارش شد. میزان شیوع سوءمصرف مواد مخدر در بین رانندگان وسایل نقلیه عمومی برونشهری ایران بالا است. پیشگیری از سوءمصرف مواد در بین این رانندگان میتواند راهبردی اساسی در پیشگیری از سوانح ترافیکی باشد. تستهای اجباری اعتیاد بخصوص برای رانندگان تریلی و کامیون و در مسیرهای طولانی باید انجام شود.
<strong>Objective (s):</strong> Commercial drivers are considered as a high-risk group for road traffic injuries (RTIs) in Iran and a clear relationship has been demonstrated for drug misuse and RTIs. To present the epidemiology of drug dependence among public transport drivers in Iran.<br>
<strong>Methods:</strong> Using capture-recapture sampling method the prevalence rate of drug dependence was estimated. Overall, 1506 drivers were randomly selected proportionately by type of vehicles. To test the drug dependence, a multi-drug rapid urine kit (ACON 10 Panel Drug Screening Test) was employed. Trained interviewers accompanied by traffic police officers in selected roads asked the drivers to answer the questions and give a urine sample at the police check points.<br>
<strong>Results:</strong> Overall, 14.1 percent of drivers were test positive. The top three drug dependence was from opium (55.0%), morphine (21.3%) and methadone (8.5%). Using logistic regression model showed no statistical difference for type of drug dependence by educational attainment. Drivers of trailers (21.5%) and trucks (15.1) were more likely to be drug-dependent compared to other drivers (p<0.001). Many addicted drivers preferred to use drugs during the night time (44.3%) and drive long distances (26.8%).<br>
<strong>Conclusion:</strong> The prevalence rate of illicit drug use among Iranian commercial drivers is high<span dir="RTL">.</span> Prevention of drugged driving among commercial drivers is an essential strategy for avoiding RTIs. Mandatory drug testing specially during the night time should be put inforce for commercial drivers particularly for trailer, truck, and for long distance drivers.<br>
