fa بررسی تاثیر اجرای طرح تحول سلامت بر عملکرد بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی شهید بهشتی در استان تهران مدیریت خدمات بهداشتی درمانی Helath Services Management توصیفی Descriptive <div style="text-align: justify;"><strong>مقدمه: </strong>طرح تحول نظام سلامت به منظور حفاظت از اموال مردم، ایجاد عدالت در دسترسی به خدمات سلامت و افزایش کیفیت خدمات در سال ۱۳۹۳ در کشور اجرا شد. هدف این مطالعه، بررسی شاخص‌های عملکردی بیمارستان‌های علوم پزشکی شهید بهشتی، قبل و بعد از اجرای طرح تحول نظام سلامت کشور است.<span dir="LTR" style="font-size:11.0pt"></span><br> <strong>مواد و روش‌&shy;کار:</strong> در این مطالعه توصیفی- مقطعی، ۱۵ مرکز آموزشی و درمانی تحت پوشش دانشگاه علوم پزشکی شهید بهشتی در استان تهران در بازه زمانی سال&shy;های ۱۳۸۸ تا ۱۳۹۸، شامل ۵ سال قبل از اجرای طرح تحول نظام سلامت (سال ۱۳۹۳) و ۵ سال بعد از اجرای آن مورد بررسی قرار گرفتند. هفت شاخص عملکردی شامل درصد اشغال تخت، میزان مدت اقامت، نسبت‌پذیرش، تعداد بیماران بستری، تعداد مراجعان سرپایی، تعداد عمل‌های جراحی و تعداد عمل‌های سزارین به&rlm;عنوان متغیرهای اصلی بررسی شدند.<span dir="LTR" style="font-size:11.0pt"></span><br> <strong>یافته‌ها: </strong>میانه شاخص‌های عملکردی به جز تعداد سزارین و میانه‌ی مدت اقامت در بیمارستان به طور معنی‌دار بعد از طرح تحول سلامت، بیشتر از قبل از آن بوده است و میانه‌ی مدت زمان اقامت بعد از اجرای طرح تحول در غالب بیمارستان‌ها با کاهش همراه بود. میانه‌ی سایر شاخص‌های عملکردی غالباً در بیمارستان‌های مختلف افزایش پیدا کرده بود. مضافاً بیمارستان‌های امام حسین (ع)، طالقانی و لقمان حکیم دارای بیشترین تعداد تخت ثبت شده بودند.<span dir="LTR" style="font-size:11.0pt"></span><br> <strong>نتیجه‌گیری:</strong> اجرای برنامه طرح تحول نظام سلامت بر شاخص&rlm;های عملکردی بیمارستان‌ها تاًثیر به نسبت مثبتی داشته است و باعث ایجاد تغییرات مثبتی در روند و میانه‌ی این شاخص&rlm;ها طی سال&rlm;های ۱۳۹۴ تا ۱۳۹۸ شده است.<span dir="LTR" style="font-size:11.0pt"></span></div> <div style="text-align: justify;"><strong>Objective (s):</strong> The Health Transformation Plan, initiated in 2014 in Iran and sought to protect public assets, promote equitable access to healthcare services, and enhance service quality. This study evaluated the performance indicators of hospitals affiliated with a medical university before and after the implementation of the plan.<br> <strong>Methods:</strong> This cross-sectional study encompassed 15 teaching medical centers affiliated with Shahid Beheshti University of Medical Sciences in Tehran province, Iran from 2009 to 2019, consisting of five years prior to the implementation of the plan (pre-2014) and five years following its implementation. Seven key performance indicators, including bed occupancy rate, length of stay, admission ratio, number of hospitalized patients, number of hospitalized referrals, number of surgeries, and number of cesarean sections, were investigated as primary variables.<br> <strong>Results:</strong> The majority of performance indicators exhibited significant increase after the implementation of the Health Transformation Plan, with the exception of the number of cesarean sections and median length of hospital stay. Individual hospital analysis revealed a decrease in median length of stay in most hospitals post-implementation, while the median values of other performance indicators generally showed an increase across different hospitals.
