fa بررسی وضعیت تجویز و هزینه آزمایش های بیماران بستری در بحران کووید 19؛ مقایسه دو بیمارستان دانشگاه علوم پزشکی کاشان Prescription and the cost of laboratory tests for patients admitted to two hospitals of Kashan University of Medical Sciences in the Covid 19 pandemic مدیریت خدمات بهداشتی درمانی Helath Services Management توصیفی Descriptive <div style="text-align: justify;">مقدمه: خدمات آزمایشگاهی علی رغم نقش اساسی که در تشخیص و مدیریت بسیاری از آسیب شناسی ها دارد، معمولاً بعنوان یکی از کانون های هزینه ی بخش سلامت مدنظر قرار می گیرند. لذا این مطالعه به بررسی وضعیت تجویز و هزینه آزمایشات بیماران بستری در دو بیمارستان دانشگاه علوم پزشکی کاشان در همه گیری کووید ۱۹ پرداخت. روش کار: پژوهش حاضر کاربردی و مقطعی بود. جامعه آماری، بیماران مبتلا به کوید۱۹ بستری در بیمارستان های دانشگاه علوم پزشکی کاشان (یک مرکز آموزشی درمانی و یک بیمارستان درمانی) در بازه زمانی اسفند ۱۳۹۸ تا اسفند ۱۳۹۹ بود. تجزیه و تحلیل داده ها با استفاده از میانگین، انحراف معیار، جداول توزیع فراوانی و از آزمون T زوجی انجام گرفت. یافته ها: یافته ها نشان می دهد در پیک اول و سوم کوید۱۹ در دو بیمارستان، ۴۲ نوع آزمایش تجویز شده است. بیشترین تجویزها مربوط به آزمایشات  Cr، BUN ، K،Na و بیشترین هزینه ها مربوط به آزمایشات Troponin و CRP بوده است. در بیمارستان درمانی تفاوت معنی داری در تجویز و هزینه آزمایشات در پیک اول و سوم مشاهده نشد، اما در مرکز آموزشی درمانی تفاوت معنی داری در تجویز آزمایشات در پیک اول و سوم وجود داشت. نتیجه گیری: با توجه به ناشناخته بودن بیماری کوید۱۹ و ابتلای زیاد مردم منطقه کاشان در شروع همه گیری، تصمیم گیری قطعی در مورد لزوم انجام تست های آزمایشگاهی امکانپذیر نبود. اما یافته ها بر این موضوع دلالت دارد که تعدادی از آزمایشات برای اکثر بیماران در فاصله زمانی مشخص درخواست شده است، در حالی که مطلوب تر آن بود که تجویز این آزمایشات موردی، با توجه ویژه به شرایط بالینی بیماران و از طریق هم اندیشی و تشکیل کمیته علمی انجام می گرفت.</div> <div style="text-align: justify;">Objective (s): Laboratory tests, despite their essential role in the diagnosis and management of many diseases, are often costly. Therefore, this study investigated the prescription and the cost of tests for patients admitted to two hospitals of Kashan University of Medical Sciences in the Covid 19 pandemic. Methods: This was a cross-sectional study on a sample of patients with Covid-19 that admitted to two hospitals of Kashan University of Medical Sciences (a teaching hospital and a general hospital). We collected the data on prescription and laboratory tests and analyzed the data using the statistical tests. Results: The results showed that 42 types of tests were prescribed in the first and third peaks of the Covid 19 pandemic in the two hospitals. Cr, BUN, K, Na tests were the most prescribed tests, and Troponin and CRP were the most expensive tests. There were no significant differences in prescribing and cost of tests in the general hospital, but in the teaching hospital there were significant differences in prescribing and cost of tests in the first and third peaks (P= 0.003). Conclusion: Due to unknown nature of Covid 19 disease and the high incidence of the disease at the beginning of the pandemic, a definite decision for laboratory tests was not possible.
